Man accused of forging medical excuses for daughter

An Oil City man is facing 40 charges after he allegedly forged medical excuses for his daughter who attends Oil City Middle School.

A criminal complaint filed through district judge Andrew Fish’s office said that between October and March, Thomas Scott III, 45, and another man created fraudulent medical excuses for Scott’s child to present to Oil City Middle School.

Scott and the other man assumed the identity of three separate medical providers of Butler Health Systems and Seneca Medical Center, the complaint said.

Oil City police were provided on Monday with the excuses from the middle school, which were then given to the practice manager at Seneca Medical Center.

The practice manager confirmed the documents weren’t authentic, further stating one of the provider’s names and credentials used on the excuses has not been employed by their facility since last summer, the complaint said.

On Friday, police spoke with Scott at Fish’s office, where he denied physically creating the documents himself, the complaint said.

Scott explained the other man involved offered to provide all the forged excuses, accompanied by an authentic excuse from the medical center, the complaint said.

Scott is facing eight felony counts each of conspiracy-forgery and forgery, in addition to eight misdemeanor counts each of conspiracy-identity theft, identity theft and corruption of minors.

He was arraigned before Fish and released on $15,000 unsecured bail.

A preliminary hearing is tentatively scheduled for Wednesday in Central Court.

The other man allegedly involved in the fraud hadn’t been charged as of Friday afternoon.
